There is a $25 dollar cancellation fee if we are not informed within 24 hours of the appointment. Please let us know 24-48 hours ahead of time if you cannot make your appointment.
If you come unprepared to your appointment then a fee may be applied to your account.
- Please Fill out the Patient Information, Medical History, and Medication List Forms within this packet and bring to the appointment. Having these filled out will help in processing your appointment at the front desk much quicker.

Medications That Affect Testing
- List of common antihistamines patient needs to be off if testing is going to be performed. We cannot garentee you will be tested upon the first visit, but this list is a list of medications that can interfere with testing.

- This form needs to be filled out every year or anytime patient changes his or her last name, address, phone number, or medical insurance. (If medical insurance changes we need a copy of your medical card.)
