We are preferred providers for Regence, Premera Blue Cross and Federal Blue Cross.
We are contracted with most managed care insurance plans. Please keep in mind that
managed care plans require that you have an authorized referral from your PCP, and be sure that an approved referral is sent
to our office prior to your initial appointment.
We accept assignment
on Medicare and will bill your supplemental insurance for you.
We
participate in the State Medical Assistance program (DSHS). Please bring a current coupon at each visit.
Please come prepared to pay any required office visit co-pay at the time of each visit.
You will be sent a monthly statement for any balance due after your insurance company has considered your claim. Monthly payments
are required upon receipt of your statement. We accept cash, checks, VISA, Master Card, or Discover.
